Assessment Form & Documentary Requirements for Itogon Tricycle Operator / Driver Applicants

DOCUMENTARY REQUIREMENTS FOR APPLICATIONS 1 to 5

1.  PROVISIONAL Tricycle Franchise (60 days):
   • Certificate of Registration (CR) and Official Receipt (OR) of the Motorcycle / Tricycle from the Land Transportation Office (LTO);
   • Proof of Common Carrier Insurance;
   • Barangay Clearance from where the applicant resides;
   • Barangay Certification on the applied place / zone of operation;
   • Police Clearance from the Itogon PNP;
   • One (1) piece of 2×2 ID Picture
   • Community Tax Certificate (CEDULA) from Barangay Treasurer or Municipal Treasurer’s Office;
2. NEW Permanent Tricycle Franchise (5 Years)
   • FOR-HIRE Certificate of Registration (CR) and Official Receipt (OR) of the Tricycle from LTO;
   • Full Compliance on the Physical Requirements (Inspection Checklist) for the Motorized Tricycle Unit.
3. RENEWAL of Permanent Tricycle Franchise (5 Years)
   • FOR-HIRE Certificate of Registration (CR) and Latest Official Receipt (*not older than 1 year) of the Tricycle from LTO;
   • Full Compliance on the Physical Requirements (Inspection Checklist) for the Motorized Tricycle Unit.
   • Proof of Common Carrier Insurance;
   • Barangay Clearance from where the applicant resides;
   • Barangay Certification on the applied place / zone of operation;
   • Police Clearance from the Itogon PNP;
   • CEDULA
4. Cancellation of Franchise
   • Letter addressed to the IMTFRB Chairperson thru the SB Committee Chair on Transportation to cancel franchise.
   • OLD MTOP Plate Number
   • OLD Permanent Franchise Document
5. PROVISIONAL Change of Unit
   • Letter addressed to the IMTFRB Chairperson thru the SB Committee Chair on Transportation to change / amend the Tricycle Franchise.
   • NEW (Private) Certificate of Registration (CR) and Official Receipt (OR) of the Motorcycle / Tricycle from LTO.
   • Copy of OLD Certificate of Registration (CR) and Official Receipt (OR);
6. Change of Unit with the Amendment of Tricycle Franchise
   • FOR-HIRE / AMENDED Certificate of Registration (CR) and Official Receipt (OR) of the Tricycle from LTO
   • Full Compliance on the Physical Requirements (Inspection Checklist) for the Motorized Tricycle Unit.
   • OLD MTOP Plate Number
   • OLD Permanent Franchise Document
7. Accreditation of Tricycle Driver for hire.
   1. For Operators
      • Professional Driver’s License to drive a Tricycle – Restriction Code A1 (Student Driver’s License will not be accepted).
      • One (1) piece of 2×2 ID Picture
      • Current Municipal Tricycle Operator’s Permit (MTOP);
   2. For Tricycle Drivers Only
      • Professional Driver’s License to drive a Tricycle – Restriction Code A1 (Student Driver’s License will not be accepted).
   3. One (1) piece of 2×2 ID Picture
   4. Barangay Clearance from where the applicant resides;
   5. Barangay Certification on the applied place / zone of operation;
   6. Police Clearance from the Itogon PNP;
   7. CEDULA
